서론
관절염은 오랜 기간의 통증과 부종, 기능 상실 등 심하게는 관절의 변형까지 초래하는 질
환으로, 일상생활 활동에 많은 제한을 준다. 미국 관절염 협회의 보고에 의하면 관절염은 가
장 흔한 불구 질환이며 미국 인구의 7명중 1명은 관절의 불편감을 호소하고 있고 Lorig은
관절염이 노인에게서 신체장애의 주요한 원인이며 숙련된 간호를 받기 위해서 입원하는 주
원인이기도 하며, 일상생활에서는 많은 불편감을 초래한다고 하였다.
환자들은 장기간의 투병 생활로 인해 불구에 대한 불안감, 사회적인 격리, 신체적인 불편
감, 경제적인 문제, 역할 상실 등 여러 가지 문제로 우울에 빠지기 쉽고 만성적인 질병으로
말미암아 환자들은 삶의 의미와 희망을 잃고, 슬프고, 무력하고, 공허하며, 투병할 의지를 잃
고, 우울에 빠지기 쉬운데 이러한 정서반응은 환자의 신체적·정신적 안녕 상태에 커다란
위협을 주고있다.
만성관절염 환자들은 운동장애의 정도도 매우 다양하여 아주 사소한 것에서부터 영구적이
고 완전한 장애에 이르기까지 심리적인 면에 많은 영향을 받는다. 특히 환자들은 심한 통증
과 함께 증상이 장기화되고 극단적인 치료 효과를 기대하기 어려운 이유로 어느 정도 치료
지시를 이행하다가 포기하는 경향이 있어 증상을 더욱 악화시키게 된다.
슬관절은 체중부하 관절로 심한 퇴행성 변화와 굴곡, 외반(valgus) 및 내반(varus) 변형이
급속히 생겨 보행에 지장이 오므로 보존적 요법이 비효율적일 때 과감한 수술요법이 요구된
다. 체중이 부하되는 관절, 즉 고관절이나 무릎관절에 생긴 심 한 골관절염은 수술을 하면
극적으로 치료될 수 있다. 관절의 양측 면이 모두 심하게 파괴되었을 때는 최소한 편측 관
절의 운동을 유지하기 위하여 슬관절 전치환술(total knee replacement)을 시도한다. 이는
골관절염에서 중요한 수술로서 실제로 수술 받은 사람에게는 수술후 통증이 사라지며 걷고
활동할 수 있게 되는데 이는 최근에 개발된 수술 방법으로 수년 전보다 훨씬 효과가 좋다.
골관절염으로 무릎관절이 완전히 파괴되어 더 이상 관절기능을 유지할 수 없을 경우 슬관
절 전치환술은 관절기능을 복원하여 정상적인 관절상태를 만들 수 있으므로 골관절염의 새
로운 치료방법으로 완전히 자리를 굳히고 있다.

This study was performed to analyze the patient's experience during the process of
disease in the patients with osteoarthritis, who are taken the replacement surgery of
knee joint.
The examine was consisted of five patients with osteoarthritis, who are taken the
replace mint surgery of knee joint from Dec. 4. 1995 to May, 20, 1996 at C university
hospital. After hospitalization, the physical and psycho logical status of the patients
during preoperation, postoperation and discharge was examined. The data were examined
according to the ethnographic method.
The results are as follows.
The patients experienced the periods of embarrasment, conflict, before surgery
suffering, acceptance period after surgery.
In the embarrasment period, the patients take a multiple medication therapy including
hospital treatment, oriental medication and folk medication to ameliorate joint pain after
first diagnoses on arthritis. The embarrasment period Includes compulsive drug
metrication, oriental medication, folk medication, trouble some, sadness and survey of
hospitals.
In the Conflict period, the patients consider the operation of knee because of working
difficulty and severe joint pain, while they feel anxiety about the surgery. They
Condemn their physical situations. They have the conflict and anxiety on surgical
operation. they consider the quality of life. They hope the surgery makes patients to
improve walking ability. This period includes self-condemned, sorry, tiresomeness,
expectation, worrisomeness, anxiety and hesitance.
In the suffering period, the patients experience post operation physical discomfort after
the total knee replacement. They do physical exercise, including extension and straight
leg raising to maintain walking ability, while they endure to wait approximately 6
months for normal walking movements and they are also unstable to environmental
people's sight. This period includes postoperative pain, continuous discomfort, inability
and communication difficulty to other`s people.
In the acceptance period the patients consider longerity of artificial joint and also
endure mild remaining joint pairs. Some of them have religions for their wellbeing of
life. This period include a self-protesting policy, abandonment, self-consolation,
dependence on religions.
According to the result from this study I suggested these shown below.
1) After replacement surgery of knee joint, continuous investigation on outcome
patient is necessary.
2) It is also necessary to analyze on patient's experiences, who are taken the
replacement surgery of hip joint.
3) Study on disease experiences of patients with rhematoid arthritis, who take drug
medication and physical therapy alone without surgery, is necessary.
4) Investigation on patient's favorable folk medication may be helpful to analyze
disease expedience of patients with osteoarthritis.
